Important Update Regarding the Distribution of Form 1095-B for Tax Year 2024
Horizon is always looking for ways to make doing business with us easier and more convenient. That’s why we’ve changed how we’re distributing Form 1095-B to our fully insured subscribers this year.
What Is Form 1095-B?
Form 1095-B reports the names, addresses and Social Security Numbers of individuals covered under a fully insured health plan and the number of months each member had coverage from January 1 to December 31 of each calendar year. You can learn more about Form 1095-B here.
How Is the Distribution Changing This Year?
In past years, Form 1095-B was mailed to all subscribers enrolled in a fully insured plan. This year, any subscriber enrolled in a fully insured health plan who registered for a secure member account via HorizonBlue.com will receive an email the week of January 27, 2025, telling them how to access their Form 1095-B online. These subscribers will not receive a printed copy of the form in the mail.¹
Any subscriber for whom Horizon doesn’t have an email address will receive a printed copy of the form in the mail.
How Do My Clients Access Their Form 1095-B Online?
To view or download Form 1095-B, subscribers must first sign in to HorizonBlue.com, then:
- On the left navigation, click on Document Center.
- Then click on Tax Documents.
Where Should My Client’s Covered Employees Go for Help or Questions?
- Incorrect form information: If your client’s covered employees see that the information on Form 1095-B is incorrect, they should call Member Services at the number on the back of their member ID card.
- Technical issues: If your client’s covered employees have an issue accessing or downloading a copy of Form 1095-B from their online member account, they can call the eService Desk at 1-888-777-5075, Monday-Wednesday and Friday, from 7 a.m. to 6 p.m., Eastern Time (ET) and Thursday from 8 a.m. to 6 p.m. (ET).
- Questions for Horizon about Form 1095-B: For answers to frequently asked questions, your clients can go to Member FAQs and search for Form 1095-B.
- Questions about Form 1095-B and tax preparation and implications: Your client’s covered employees should consult a tax professional or the IRS for any questions regarding their personal tax situation or how to use the form to prepare their tax return. Horizon cannot provide tax or legal advice.
