IHC and SEH Product Portfolio Updates for Plan Year 2025

Horizon is updating its Individual Health Coverage (IHC) and Small Employer Health (SEH) product portfolios for 2025. Please read this Brief Notes for important information on health plan changes.

Beginning November 1, 2024, Summary of Benefits & Coverage (SBC) documents for our 2025 IHC market health plans are available through our online shopping experience and when you sign in to HorizonBlue.com/brokers.

IHC subscribers will receive a 2025 SBC for the plan that they are currently enrolled, which will include any applicable changes. If subscribers switch plans, they can view the SBC for their new plan at the time of enrollment, and for on-exchange subscribers, a new SBC will be mailed per on-exchange requirements.

Small employer customers who offer a health plan modified for 2025 will receive two SBCs — one for their 2024 health plan that is effective until the time of their renewal, and one for use on and after their renewal that includes the modifications to the plan effective upon renewal.

Advantage EPO Health Plans with BlueCard ®
With these plans, members have access to all doctors, hospitals and other health care professionals that participate in the Horizon Managed Care Network. With BlueCard PPO, members outside of the Horizon service area do not need to select a Primary Care Physician (PCP), and referrals are not needed to access specialty care. Members can access any network doctor or hospital at any time and enjoy the in-network level of benefits.

Members benefit from lower out-of-pocket costs when care is coordinated through a selected NJ PCP or when BlueCard members are seen by specific provider specialties (Internal Medicine, Pediatrician, Family practitioners).   

How to Access to Care
Our members have access to doctors and health care professionals, and hospitals in convenient locations across New Jersey and parts of Pennsylvania and Delaware. Members can find in-network doctors, hospitals and other health care professionals by searching our Doctor & Hospital Finder at HorizonBlue.com or on the Horizon Blue app.
